Short answer

Zepbound self pay costs $299 a month for the 2.5 mg dose, $399 for 5 mg and $449 for 7.5 mg to 15 mg through Lilly's self-pay offer for the KwikPen, if you refill within 45 days, and Walmart pharmacies are a pickup option for LillyDirect. With commercial insurance that covers the single-dose pen, the Zepbound Savings Card can bring it to $25 a fill through December 31, 2026, and eligible Medicare patients pay $50 a month for the KwikPen.

Zepbound self pay cost

Zepbound comes in three forms: a single-dose pen, a multi-dose KwikPen and single-dose vials. Lilly’s self-pay offer covers the KwikPen and vials, not the single-dose pen, and you can fill it through LillyDirect Pharmacy or another pharmacy.[1] The table above shows the price for each dose.

The $449 price for 7.5 mg and up applies only when you buy your refill within 45 days of the previous delivery; you get it automatically on your first purchase of those doses.[1] Lilly’s $299 offer covers a 1-month prescription of the KwikPen or 4 vials, and Lilly’s page lists the $399 and $449 tiers only in the KwikPen terms, so confirm vial prices for higher doses with LillyDirect.[1] TrumpRx lists Zepbound at $299 a month against an original price of $1,087.[2]

Zepbound cost by pharmacy

Where you fill changes what you pay without insurance:

Where How it works Price per month
LillyDirect home delivery Self-pay offer, KwikPen or vials $299 to $449 by dose, with on-time refills[1]
Walmart pharmacy Pickup option for LillyDirect self-pay orders[6] Same LillyDirect tiers, per an October 29, 2026 check[7]
CVS pharmacy CVS says eligible Zepbound patients can see cash-pay prices in the CVS Health app from early in the fourth quarter of 2026, with same-day pickup at about 9,000 stores[9] No posted price found; check the app or ask for a quote[7]
Other pharmacies Lilly lists “other pharmacy” as a self-pay option[1] Ask for a quote

A second price check found Zepbound left off Costco’s GLP-1 member pricing list, which covers Novo Nordisk drugs.[7] Ask any pharmacy for the total before you pay, and compare it with the LillyDirect price.

Zepbound Savings Card and cost with insurance

If your commercial plan covers Zepbound, the Zepbound Savings Card lowers what you pay for the single-dose pen:[1]

  • Pay as little as: $25 for a 1-month, 2-month or 3-month fill
  • Maximum savings per fill: $100 for 1 month, $200 for 2 months, $300 for 3 months
  • Maximum savings per year: $1,300
  • Expires: December 31, 2026
  • Who qualifies: commercial drug insurance, age 18 or older, U.S. or Puerto Rico resident, not enrolled in Medicare, Medicaid or another government program

If your copay is larger than the card’s limits, you pay the rest. If your plan does not cover Zepbound at all, the self-pay KwikPen price is the cash option.

Zepbound with Medicare

The Medicare GLP-1 Bridge covers the Zepbound KwikPen only, for $50 a month. The single-dose pen and vials are not covered. The $50 does not count toward your drug plan deductible or yearly out-of-pocket limit.[3] The Bridge runs July 1, 2026 through December 31, 2027.[4]

You need Part D coverage, must be 18 or older and must meet BMI and health condition rules. People with moderate to severe sleep apnea, type 2 diabetes or fatty liver disease are not eligible for the Bridge and should ask their drug plan, which may already cover a GLP-1 for them.[3]

What Zepbound is approved for

Lilly describes Zepbound as an injectable prescription medicine that may help adults with obesity lose weight, and that may help adults with moderate to severe obstructive sleep apnea and obesity improve their sleep apnea.[5] It contains tirzepatide, the same ingredient as the diabetes drug covered in the Mounjaro cost guide. Your clinician decides whether it fits you.

How to pay less for Zepbound

  1. Check your plan’s coverage. If it covers the single-dose pen, the Savings Card can bring a fill to $25 until December 31, 2026.[1]
  2. Refill on time. For 7.5 mg and up, refilling within 45 days keeps the KwikPen at $449 instead of $499 or $699.[1]
  3. Check Medicare GLP-1 Bridge eligibility. If you qualify, the KwikPen is $50 a month.[3]
  4. Compare with other GLP-1 drugs. See the cheapest weight loss injections guide for cash prices side by side, or the Wegovy pill cost by dose.
