Doctor visit cost by visit type
Office visits are billed by code. The code depends on whether you are new to the practice and how complex the visit is, and the table above gives CoveredUSA’s 2026 cash prices for each code, based on CMS, FAIR Health and other data.[1] New patient visits cost more because they take longer. For a reference point when you ask an office for its cash price, Medicare pays about $110 for a routine new patient visit (99203), $165 for a complex one (99204), $90 for a routine established patient visit (99213) and $136 for a complex one (99214).[1]
Hospital-owned clinics cost more
A visit at a hospital-owned clinic can cost $200 to $600 or more, because the bill includes a facility fee of $150 to $500 on top of the doctor’s fee.[1] Ask whether a practice bills a facility fee before you book.
Lower cost places to see a doctor
- Community health centers (FQHCs): $20 to $80 a visit on a sliding scale based on income. Patients at or below the federal poverty level often pay $20 to $50.[1]
- Retail clinics and urgent care: about $80 to $200 a visit.[1] The urgent care cost guide breaks down test and X-ray charges.
- Telehealth: $40 to $90 a visit for problems that do not need an exam in person.[1] See the cheapest telehealth options without insurance. Care Anytime’s Virtual Physician Access benefit gives members phone and video doctor visits 24/7, with all visits included in the membership.
- Direct primary care: a monthly membership of $50 to $150 for an individual, instead of per-visit fees. From January 1, 2026, HSA funds can pay up to $150 a month for an individual or $300 for a family.[1]
- Membership primary care: Amazon One Medical lists membership at $99 a year for Prime members and $199 a year for others.[3]
Physical exam cost without insurance
Self-pay prices for a physical vary by office. CVS MinuteClinic lists camp and sports physicals at $82 and DOT physicals at $150, and does not take insurance for these.[2] With an ACA-compliant plan, an in-network preventive exam costs $0.[1]
Extra charges to ask about
Lab work and other services are billed on top of the visit. CoveredUSA lists a basic metabolic panel at $30 to $100, a CBC at $10 to $50 and a lipid panel at $20 to $80.[1] See the blood test cost guide for direct-to-consumer lab prices.
Will urgent care bill me later?
Usually you pay at the visit. Most urgent care centers offer 10% to 30% off for paying at the time of service rather than being billed later, and the discount is often not advertised, so ask at the front desk before you register.[6] Some charges can still arrive later: if a blood or urine sample goes to an outside lab, that lab bills you separately, and the bill may come days or weeks after the visit.[6] If you get a large balance, most urgent care centers will set up a payment plan rather than send the bill to collections; ask before it becomes overdue.[6]
How to pay less for a doctor visit
- Ask for the cash price up front. CoveredUSA says self-pay discounts of 20% to 40% off the full charge are typical.[1]
- Get a good faith estimate. If you are not using insurance, the provider must give you one when you ask or schedule at least 3 business days ahead.[4] If the bill is at least $400 over it, you can dispute the bill within 120 days for a $25 fee.[5]
- Use a community health center. Fees start at $20 a visit on a sliding scale.[1]
- Book the right visit length. Routine visits for established patients cost less than complex or new patient visits.[1]
